UI question

Hi, anyone knows the procedure for unemployment benefit? The form needs to put "the person name of the immediate supervisor". I wonder if the EDD (employment development department) needs to contact to this person? If this person isn't working in the company any more, then how the EDD is going to do? Thanks.
