If it is in network, then dentist only charge you the portion that you are responsible. The other portion is paid by insurance directly.

Depend on the type of coverage you have. Not all the dental program provide with free unlimited extraction.

Most dental program only covers $1500-2500/ year with very specific terms. If Dr. has to break down the teeth for removal, you will get a higher fee and the difference is not covered.
