I think your ID has been stolen by someone.
Per what you said, your name is pretty unique, so name duplication should not happend often, and especially someone who lives so close to you.
From you credit report, your credit card information, your previous address were showed on the report too, that is another alert for you.
You don't need worry too much for the credit repot. You got that name and SSN from your insurance company. It is natural for you to request the credit report. I think you should collect the evidence for your credit card, previous living address, and go to Secoial Security B to report your concern and ask them to clarify.
If I were you, the first thing I want to do is to that address, to check if there is another me at that house.
