If the doctor is outside of insurance network, the insurance policy is to pay 70% for out of network service. This doctor charge patient for procedure A, say $3K; while inside network doctor will charge $2K for the same procedure. So, the insurance will pay the out of network doctor $2.1K? While the insurance only needs to pay inside network doctor at most $2K?
