Don't bring up the medication side effect

issue. Our standard reply is: pt. did not report to me that there is such side effect (just like you said people react to medication differently). Otherwise I could switch medication for pt. Pharmacist explain drug info to pt at details as well. So no use to blame physician or pharmacist
