將W7 + 1040 寄往W7所指定地點申請. Don't mail second copy of 1040. As long as you follow the instruction on W7, you will be fine. IRS will assign and notify you the new ITIN by mail. You probably will get tax refund first. A mail for ITIN might come much later(a few months later).
