如何要回不該扣的聯邦稅和州稅?

本人H1
2004年底自大陸來美
去年2005年11月換單位時
新單位不明白情況
扣了我的聯邦稅和州稅

當時單位說
報稅時可以要求返回
但不知道如何填寫?
1040好象沒有這一項

請問
我該用什麽表格要回稅款?
