Since you hold H1B for 11 months, >>183 days. You will file Form 1040EZ, 1040A, or 1040. Read 1040 instruction or Pub 519 for more info.
2. You can claim your one-month F1 income (not $5000) as tax treaty. There is no line on 1040 for such tax treaty, you can mark on line 7 "-$ (F1 income) per article 20" and put the rest of your income in line 7 as your taxable income. Probably you need attach form 8843.
