Help needed for ITIN

I want to apply an ITIN for my father who has stayed 8 months this year.

a few confused things when filling w-7:

1: visa expiration date: in the visa in his passport, it is six months from the day in January that he was interviewed in China. He entered in May and got extended to May 2006. so which date I should use?


2. do I need to send i-94, both old and new one? the bank did not notarize copies of I-94s since they said I can ask INS to send other copies if lost.
