It was actually my friend's parents. They were on B2 visa. They were denied for the visa because they did not bring evidence that they left the US during their valid stay.
Otherwise, just imagine the following secenrio: you got an I94 for 6 month stay in the US, but instead, you stayed in US for two years (so you were out of status for 1 and half year). Then you left US. After that you are trying to apply for a visa. Consulate Officer has the very reason to think that this might have happend unless you prove otherwise.
If you do leave US within your legal stay in US, you need to prove it (contact airline company to provide a proof if you do not have boarding pass anymore)
