It is not going to be easy. I am not sure how serious his mental problem is. As you said, he is not following your advice, if you end up making decision for him, you got to have proper authorization, either a durable power of attorney executed as of the time he was still competent, court appointment of you as his guardian (but the court must find him incompetent first), or other types of authorization. Without a court order, I don't think the police will help you restrain your brother's freedom.
