You Are Hereby Notified that xxxx has retained and employed me as her attorney, and has placed in my hands for suit or collection, a claim or cause of action for injuries and damages which she sustained on or about the 15* of March, 2024 at your rental property located at xxxxx 

 

And she has agreed to pay me, and I have agreed to accept, as my attorney fee for services rendered, and to be rendered by me in the matter, a sum of money equal to one-third percent of any money or other valuable thing which may be received or paid on account of said claim or cause of action.

And You Are Hereby Notified that by means of the facts above stated, and of the statute applicable to such cases, I hereby claim a lien upon said claim or cause of action and the proceeds, and avails thereof, for the amount of said attorney fee.
