I have the same problem

One time I went to the hospital emergency, I gave them my insurance card, at last the insurance company paid the hospital bill. but the doctor did not give me bill, he send the bill to the collection company directly, and collection company asked me to pay the bill. I gave them my insurance company, they never contact the insurance company, but they
put me on bad credit. I called the hospital, the hospital said the doctor already left.

Now I still have bad credit record.
