With respect to your obligation under the authorization form -- to prevent fraud by auto repair shop --- different states have different requirements on the contract. To be binding, auto repair contract should at least include terms like estimate, cap on expenses, time for service, location for pick-up, mechanic's lien, and stuffs like that.
If these are the only terms on the form, I doubt you have any contractual obligation.
