This statement of information is updating any informaiton change of the company, including CEO change.  Also, draft a resignation letter and send to other members and keep record. 

Keep in mind that any new CEO has to be appointed per operating agreement of your LLC. 

Ask the bank what they need.  Business bank account should not be under your personal name.  They should be under the business name, and you (or any manager) should only be "authorized signer", not "owner".
