I did a little research and registered a company recently. It is really simple.

1. I think LLC should be better for you.
2. You can find a lawyer or a company which does this kind of business to file for you. One of important file is Article of Organization. You may need a register agent. A registered agent is an individual (or a corporation) designated by you to accept service of process if a lawsuit is filed against you. The register fee is low in some states. Actually you can do it yourself :)
3. Yes, you can. If your company expects to transact business in state B, your company may be required to qualify as a "foreign LLC". A foreign company can obtain a Certificate of Authority from the Secretary of State (in state B).
