請教孩子辦護照的問題， 到哪裏能拿到certified custody order?

終於離婚了，孩子的父親放棄了所有的父親權利，簽署了terminate agreement.我擁有孩子所有的監護權。兩周前去給孩子辦理護照，今天接到一封信，要求： Submit a certified custody order.

這是什麽證件，去哪裏能拿到呢？我隻有離婚協議書和父親放棄權利同意書。
