AR-11 change address

I am in 485 pending and need move, where should I send the AR-11 change address card? to USCIS Vermont (where I submitted my 485) or to the address listed on the bottom of AR-11 (a different address).

Should I attach a copy of my 485 receipt in addition to the AR-11?
