You need to ask someone go to the little town's governmental office(Xiang-Zhen Ban-Gong-shi) where the records of your birth date and your parents names can be found. Make a copy and then go to the county's noterization office (there should be one like that, otherwise go the city's office) and certify and translate it into English. Remember, it has be like that. Mr.XXX's parents are YYY/ZZZ(chinese English). Or Mr. XXX was born to YYY(Mother first) and ZZZ(father).

The date should match the date on the passport.
