Employer

I have a question regarding providing the employers
of last five years on the biograph form.
I had three on-campus jobs when I was in school.
(for different time period)
1. Student Helper (washing test tube, 15hours/week)
2. Student Helper (sorting books in the library, 10hours/week)
3. Graduate Research Assistant (University paid tuition
plus stipend each month, 20hours/week)
I am wondering that should I include all three
in the biograph form or just the last one?
