How could it happen like this? That is weird. Are you sure you put the correct birth information in all your application forms? Probably there are some discrepancies in your forms. Really I have no idea about this situation. My 485 is still pending. I did the same way. It is unusual to request such evidence. How about send the original notarized documents to CIS and provide a self-addressed and stamped envelop for returing?
