I-485填表請您把把關

請問前輩，我正在準備I-140和I-485一起file EB2NIW. 同時我還替我的太太file I-765. 填表時我有幾個問題沒把握，敬請前輩把關：

Form 750b1:
10.b. Alien is in the United States and will apply for adjustment of status to that of a lawful permanent resident in the office of the Immigration and Naturalization Service at
City: State:
這裏的城市是填我居住的城市還是遞交申請的移民處地址？

Form I-765:

Eligibility under 8 CFR 274a.12 ：我太太目前是用J-2拿的工卡，原來填的是(c) (4).
現在和我們的I-485一起file, 性質應該變了，是否填(c ) ( 9 ) ？另外，在
I am applying for: 一項，是否應該選“Permission to accept employment.” 而不是“
Renewal of my permission to accept employment“？

a. an immigration giving me an immediately available immigration visa number has been approved….

h. Other basis of eligibility

我可以選a嗎？如選b，怎樣填好？
