Time is critical here.
If your extension came first, even one day earlier than the waiver, you are OK.
If your waiver came first followed by extension, then for sure you are subject to another two-year rule bound to the new j-1 programe.

From what you wrote, it seems your extension came first because you still have not got the recommendation letter.
