did anyone get I-485 rejected

with I-140 approval. How many times are you allowed to provide additional documents? My lawyer told me that I only have one chance to provide what ever is asked. If it is still not good, then my case will be rejected. If so, is my I-140 approval still valid? Thank you very much.
