If you are just transferring your H1B, once you receive the acknowledgement of receiving (note, not the final transfer approval) your application from INS, you can start working for the other employer.

As for whether you need to leave US or not, I am not 100% sure, but sounds to me that you don't have to. At least, there is a grace period of 90 days. You can call your lawyer who did your H1B application to make sure.
