Based on the description provided, it seemed that some of USCIS expected evidence were not clearly labeled and therefore not easily identified since your lawyer put so much unrelated materials. Some key evidence should have been labeled independently but they were put together with some labled unrelated materials.
I did not see what your cover letter looks like. That is what I guess.
And also at the bottom or top of pages, you should add your name and SSN number as reference. If you have a bunch of evidence to support one claim, you should bind them together and put a list as a cover sheet. You need categorize your evidence. That will make your evidence is easily identified.
