request another FP

from local CIS office for both you and your father. You may have to stop by local CIS office in person to get requests. After you have done FP, send a letter with the list the errors along with copies of each FP receipts (circle the mistakes on the copy to CIS) to CIS to request their correction. I had similar experience before 485 approval.
