Form I-134的填法（NIW Base）

NIW,file I-485 ourself.填I-134 Form.
請教如下幾個問題：
１．需要support 我和son.
２．第３項：
That this affidavit is exexcuted on behalf of the following person:
下麵填寫我的information.

Name of spouse and children accompanying or following join person:
Spouse:一欄寫我的名字？？？
　　Child:一欄寫兒子的名字？？？
３．第８項：
　　寫我和兒子的名字？？？
４．第１０項如何填寫？
５．第１１項如何填寫？

　　另外，我現在是H4,還沒有工作。wife 是主申請人。我沒有工作是否影響I-485的成功機會？謝謝！
