go to irs.gov ,download I-539 form .prepare your supporting document according to the instruction.the extra doucument your need to send are:
1. your lattest I-20(show you applied OPT)
2.copy of your EAD(if you have)
3 copy of your spouser's I-94,passport pages.visa stamp.
