To remove the condition on your green card, you need to fill in form I-751 and submit it with evidence (documents) showing both of your names, such as Federal Tax Return,bank statemens, utility bills, grand deed or rental agreement, child birth certificate (if applicable), photos...etc. Usually the CIS approves the petition and mails the approval notice to you, requesting you make an appointment to get the permanent card done with the officer. In very rare situation, you will be asked for an interview (your petition will be transferred to the local CIS office).
