Sorry that I can not input chinese now.
As your questions, our experience is:

1) we did not notarize the work experience in china. Just the letter from the supervisor.
2) Yes, one for each, however, I don't remember what is section A about.
3) there is a check List, i think, we did not submit fingerprint and landing fee.
