Immigration to Canada: Child?

My H1 will expire in 2 years and LCA is still not processed yet. I want to apply Canada Green card as a back up. Can someone tell me should I include my daughter in the application to Canada? She was born in U.S.A. If I do not include her in the application, does she need a visa to enter Canada with me? Can she have health insurance in Canada? Thanks
