Maybe they can get one next time. They will be granted visa eventually, we just can't say how many times (0,1,2,or 3) VO wants to deny them to punish for their un-disclosed intent of extension during their last successful B visa interview. For people whose B extension was truly not anticipated (such as having a health condition in US and couldn't travel under doctor's order), there should be no penalty from VO.
