In left corner of the first page of your IAP-66 form, you will find there are some optional terms and onbe of them was marked by the immigration office. If it is two year home country residency applied, you have to get waiver otherwise you need return your home country after three years. If your program was supported by government money such as NIH fellowship, you cannot get your J-1 waiver for two year home country residency. All others shouble be no problem for application of waiver.
