J1 waiver form questions

I am applying the case number for my J1 waiver. There is a question I met when I filled out the data sheet.

On the item 11, list all the program I have had as J1.
there are three lines like

_________ ____________ ______________________________

How should I fill them?
also I have transfered four times. There are only three lines. What should I do?
