If you have been admitted into graduate school which required 疫苗, you just give physician the records of 疫苗, that is enough. That is my case (less than 2 year during whole PR processing)and I just received green card ( plastic). otherwise, you have made shots 1,3,4,5,6,7 of Supplemetal Form to I-693.Onne shot cost some $40 in Y2K in my school but I got shots in state health department which costs $5 per shot for poor students.
