Apply NIW myself,need help!

Want to apply NIW by myself, but a lot things I still unclear. Wish some persons with the experiences give me some advices. Thanks a lot!

1. If I file I-140 and I-485 same time, what other FORM do I need to file? and where can I get all these FORMs?

2. What about physical examination? when to do that? Is there any special hospital or Doctor for that?

3. What about finger print? when to do that?
