回複：看到前麵有人問綠卡體檢，大夫複印medical record

doctor should give the exactly copy of exam sealed and mailed to INS, please read the form instructions and our doctor gave the copy to us.

The doctor/INS just need copy immuninization record, not original one.You should keep the original one.

This is our case and got passport stamped last month
