I need urgent help from J-waiv

Hi,all J1 waiver,
I need your help. My no-objection letter has been sent to DOS at Oct.10,but on Dos website it still shows "request".Maybe it won't update so quickly.My situation is that I am going to get the extra half year extension from my current boss after having stayed here for three years because I do not have enough time to apply for H1 now.My status will expire after this month and I can not get the new IAP-66 form until the end of this month because of my boss's fund problem.
Should I notify DOS that I am applying for the extension before I send the new IAP-66 form to DOS in order to avoid the second waiver?
The last three IAP-66 forms have been sent to DOS when I applied for the case number.
I am really worried about the DOS will approval my case before receiving my new IAP-66 form.
