-There is not limit format. Normally, the visa officer should have indicated why your visa pplicaiton was denied in the denial letter/form, e.g., INA section 214(b), which means you did not establish eligibility for the visa category being applied for or overcome the presumption of being an intending immigrant.
You just need to put the denial reason from the the letter/form into the new DS-160, e.g., put "INA section 214(b), I did not convinced the visa officer that I did not have the immigrant intent".
