我是綠卡，而且結婚後姓氏改成了丈夫的姓氏。近期要出境遊，已經取得該國簽證。由於本人的所有受雇證明、銀行資產等都是婚後姓名，為謹慎起見，在本州州務卿做了結婚證書認證，上麵是唯一新舊名的證明。
還需要簽證國大使館認證還是中國大使館認證還是都不需要了？

Is your old name in your current passport and in your green card? Did you use your old name to apply for the visa? If yes for these two questions, you should be OK to travel by using your curent passport, green card and the visa.
But you had better also go to the court to ask the judge who approved your name change to issue you the name change certificate and bring it with you in case you may be questioned by the IO at the port of entry when you re-enter US. You may also need this name change certificate in the futrue for some other purposes, anyway.
