1. What is your USCIS Online Account Number?
2. Where did you apply for your immigrant visa or adjustment of status?
3. Where was your immigrant visa or adjustment of status issued?
I applied for mom 11 years ago and I didn't keep any record to answer these two questions. I guess the answers for the two question shall be the same, right?
-Yes, normally it should be the same, but sometimes the case may be transferred from one service center to another service center to process and approve. But it is fine to put the same service center for both questions.
However I don't know how to get the accurate information for these questions. Can I leave them blank?
-You had better fill it. Check the line of a series of number, letter and marks at the bottom of your green card (front side) to see which of the following USCIS Service Center codes in your green card:
EAC or VSC: Vermont Service Center
LIN or NSC: Nebraska Service Center
SRC or TSC: Texas Service Center
WAC or CSC: California Service Center
NBC: National Benefits Center
MSC: Missouri Service Center.
Then, put the center' name there. For example, if you find SRC or TSC in the line of a series of number, letter and mark, then, just put "Texas Service Center" there.
will it affect the renewal time if I don't fill in?
Will it be worse If I fill in with not accurate information from my memory ?
