1. page 1 is the original medical certificate of birth in Chinese with seal/red stamp
2. page 2 is notarial certificate of page 1 in Chinese with seal/red stamp
3. page 3 is the translation of page 1
4. page 4 is the translation of page 2, which certify the photocopy "confirms to the original, and that the original document is authentic."

It indicates the applicant's name, gender, date of birth, ID number, place of birth, and both parents' names. 問題是page 1 和 3上有父母名字，公證頁(page 2 和 4)隻是the notarial certificate of page 1 和 3，沒有列出父母名字，請問這能滿足要求嗎？謝謝！
