我給父親申請移民，要求上傳到National Visa Center的文件有兩樣總通不過，一是我的出生證明，我一直用之前在國內辦的出生證明，上麵寫父母的姓名，怎麽就不行了呢，說要把每個side都掃描，我也掃描了

-Probably your birth certificate 公證 is too old, not meet the requirements. You may ask NVC to see what the problem is, and you may need to do a new 出生證明公證 which includes the translation version.

 

再就是我母親5年前去世了，需要提供我母親的死亡證明。

-This document should not be required. Did NVC ask for this document?

我用的醫院給我父親出局的死亡證明，我在美國找了個翻譯以後certified translate了一下。

-Probably the translation does not meet the requirement. See this for the USCIS requirement for translation:

You may ask your friend to do it for you following the instructions in above link if this document is needed by NVC.
