• Form G-1450 (Authorized total amount of $ 1,760.00. The amount includes $1,140.00 I-485 filing fee, $85.00 biometric fee, and $535.00 I-130filing fee)
• Form G-1145 (E-Notification of Application/Petition Acceptance)
• Form G-325A (Biographic Information of the Beneficiary)
• Form I-130 (Petition for Alien Relative)
• Form I-485 (Application to Register Permanent Residence or Adjust Status)
• Form I-864 & I-864A (Affidavit of Support)我是無業，所以需要老公填寫864A
• Copy of notarized Birth Certificate of the Beneficiary
• I-94 and travel history (Admission Record printed from the website of the Department of Homeland Security)
• Copy of Beneficiary’s Passport (Biographical page)
• Two (2x) passport-style photos
• Form I-693 (Report of Medical Examination and Vaccination), in a sealed envelope
Packet B: Supporting materials for the Beneficiary’s application:
- Copy of Petitioner’s Naturalization Certificate
- Copy of notarized Birth Certificate of the Petitioner
- Copy of notarized marriage certificate of Beneficiary
- Copy of notarized No Criminal Record of Beneficiary
- Petitioner Spouse’s employment verification letter
- Federal Tax Return (e-filed) for the last 3 years (2016-2018)
