請問8老師： 485 填表

My case is：I am green card holder and I am filing 485/131/765 for my 4-year son (I filed 130 for my son and the 130 was approved and the PD is current now). I completed all forms for him.

Here are my questions:

1. 485 form,

    Part 10: Applicant’s statement, contact,… and Signature”
        1a, “I can read and understand …”    DO I NEED TO CHECK THIS?
         2 “at my request, the preparer name in part 12 ___prepared this application for me ..”  DO I NEED TO PUT MY NAME HERE？
    Part 12: Contact Information, …., of Person Preparing this Application, if Other than the applicant”  DO I NEED TO COMPLETE THIS?

2. I-864 form:
For “IRS tax return transcript”, DOES IT MEAN THE “transcript we print from IRS website” ?  I just login the IRS website and  print the copy of the transcript from IRS website, IS THIS ENOUGH? DO I NEED THE COPY OF 1040, W2 WE FILED TO IRS?
