高齡父母持綠卡六年左右，最近想申請入籍。父親前幾年小中風，語言和記憶都不大行了，需要申請免試。請問申請公民時必須同時遞交648表格嗎？
-Yes if you want to get the waiver.
一般的家庭醫生可以填這個表，還是必須專科醫生才行？謝謝
-Probably your father has to conduct the physical exam by the licensed physicians (專科醫生) and then, the doctor will complete this form for him.
