H-4, Thanks again!

Please give me a help, I want to know the follwoing informations in detail:

Is it easy or difficult that my wife and my son apply H-4 visa after I get the H-1B? How long time it is needed? And it still has to apply in USCIS? And extra 1000 $ should pay for the premium processing?
