請教8老師： H4 EAD 申請材料

8 老師您好： 

我在USCIS網站上看到以下證明身份的材料清單 （https://www.uscis.gov/working-united-states/temporary-workers/employment-authorization-certain-h-4-dependent-spouses#How%20to%20Apply）：

我不確定要求中“OR" 的意思：

  • 是需要提供1&2&3 項 加上 4或5 中的任意一項？
  • 還是隻提供5項中任意一項即可？

另外請問第4項：美國使館發的H-4 簽證可以嗎？

還有請問第5項：是指中國身份證嗎？

Evidence of :

A government-issued identification document with photo
  • A copy of your last EAD (if any);
  • A copy of the biometric page of your passport;
  • A birth certificate with photo ID;
  • A visa issued by a foreign consulate; or
  • A national identity document with photo.
