I-90 part 3 入境問題請教

上次說母親I-90被拒，我準備從新發一新的I-90.  不明白3.a. 的問題。母親是B-2入境，留在美國後，我給辦的I130/I485 co-current filing. 她需要添3.a 嗎？

由於要把有錯的綠卡寄回去。我還能做 online filing 嗎？還是做paper mailing with original green card
